Answer: Think of it like this. The banks hold all the money and the distribute all the money. When you put your money into the bank say a saving account you usually recieve less than 1% return, if its a CD you might get at the most 4% if you are lucky. Yet, the bank then goes and give the money you just gave them out as loans: morgages, car loans, credit card charging those people any where from 6 - more than 20%. So that extra 19% is all profit for the bank. Not to mention on top of the intrest rates the charge the loan holder there are fee's involved monthly and annually, you are also charged in some banks fees on your savings account. Nowadays, LASIK eye surgery has become more and more popular among eyeglasses wearers. Thus, one may wonder what the merits and demerits are in getting this surgery. This question is very important for people who want to receive such surgery. As a receiver of LASIK surgery, I have my own experience. Based on these experiences, I would like to let more people know the facts.

The merits are:

1. You can enjoy normal vision

2. It is no need to wear glasses any more

I can not list more demerits except the above three. The surgery is really of magic power. You may not believe it. Now here is my story. When I was 5 years of age, I can not live without glasses. Later, I had received LASIK surgery when I was 22. Till now, 7 years has been passed. I can still enjoy the perfect vision and brightness brought by the surgery. With the process of aging, the elasticity of eye muscles will decline. Such process is irreversible. Anyone who is over the age of 40 has to wear reading glasses. If you are worrying about this, just go and get LASIK surgery. Generally, the merits of the surgery exceed its demerits. Hence, you can now contact with the LASIK center, consult with the doctors and do some exams. The doctor will tell whether such surgery is suitable or not for you.

Community General Hospital (a fictitious institution) is a 250-bed hospital located at the periphery of a major metropolitan area. Community General's revenues were flat for the three years ending 2001, and with rising expenses, the hospital's operating deficit increased from $2 million in 1998 to $5 million in 2001. The hospital's financial managers applied every measure they could think o[ to control expenses and increase revenue yield, but their efforts only slowed the rate of financial decline.

At the end of 2001, the board decided to replace the CEO.

The new CEO declared that the hospital had a revenue problem and embarked on an aggressive set of initiatives to expand revenue, based on the premise that with the hospital's high fixed costs, almost any incremental revenue would yield contribution margin and, more likely, net margin. Revenues grew from $100 million in 2001 to $125 million in 2003, but operating losses rose to $10 million as well.

Again, the board elected to fire the CEO, this time along with the senior financial executives. What went wrong?

Lessons Learned

This fictitious case study highlights three important lessons that hospitals should keep in mind when seeking to balance new revenue growth with controlling expenses.

Start by getting the house in order and have a solid foundation on which to grow. As all good financial managers know, there's no substitute for sound fundamentals. Community General may indeed have had a revenue problem, but it also had an expense problem. The second CEO could and should have found ways to reduce costs further.

Sound easy? Most of these recommendations are simple business discipline essential for successful growth, but surprisingly lacking in today's healthcare industry.

"We are told he was Asia's heaviest man when we operated on him 10 days ago," announced Dr. Muffazal Lakdawala, relating how he was invited to China to place a gastric band around Qin's stomach. The Chinese man has since lost seven kg, Dr. Lakdawala said at a press conference held in L H Hiranandani Hospital, Powai, on Friday.

The Hiranandani team was first asked to operate on a 25-year-old-woman who weighed 170 kg during the workshop. "The Chinese doctors said that they had so far only operated on 130 kg patients," said the doctor. "After the girl's operation, we were introduced to Qin as the heaviest man in Asia."

Now, the Chinese doctors want to train at Hiranandani Hospital on the nuances of bariatric surgery. Given a recent survey's finding that 25% of Chinese men were obese, this is not surprising. "Batches of four doctors will come in turn to train at hospital early next year," said medical director Dr Sujit Chatterjee.

Bariatric surgery is used to reduced the size of the stomach of a morbidly obese person or one whose body mass index is over 37 (BMI is equal to one's height divided by one's weight). Doctors either place a band around the stomach or a make a bypass between a portion of the stomach to the small intestine to reduce the effective area of the stomach. The logic being that morbidly obese patients will then carve less for more food and start losing weight.

"Even in anaesthesia, the doctors concerned were preparing doses that were too heavy for the patient," said Dr Sunita Goyal who was part of the team that visited Tianjin last fortnight. Chinese anaesthetists, too, are also planning to come down for training to Mumbai.

Dr Muffazal Lakdawala believes the Chinese experience is a pointer that bariatric surgery will boost medical tourism to India. "It proves that we have the knowledge and the infrastructure to carry out such a surgery," he said, adding that he has been invited to Sri Lanka for another workshop.

Moreover, given the fact that obesity surgery costs a fraction of the costs abroad – $7,000 in the city as against $33,000 in the West–India makes economic sense.

Michael Carluccio's fans lately are singing a different version of Darin's hit:  "Look out, Detroit, Mikie's back in Motown!"  Back in town with his 1-Man Legend Show, Mike has been entertaining audiences of all ages in his home state and reprising the fabulous, familiar music of the 50's and 60's.

Singer, actor and action-hero impersonator, Carluccio's 1-Man Legend Show is a huge hit with audiences of all ages.  Recreating the swinging, devil-may-care, cool, rat-packish style with his songs and one-line adlibbing humor, the 1-Man Legend Show features Sinatra, Dean Martin, Bobby Darin, Neil Diamond, Tom Jones and others.

Resting backstage from his latest performance, Michael welcomes the close interaction with audiences.  "I love the applause and fanfare," confessed Mike.  "But, I get a kick out of the smiles and tears on the faces of the audiences as they reminisce to their music."

By day, Michael dons the makeup and costume for his alter-ego trips as an "action-hero" impersonator.  Bearing an uncanny resemblance to many action-hero characters, Mike has been seen around town lately as the leader of the X-Men, the Wolverine.  Family style restaurants to charity events hire the Hugh Jackman look-alike to switch from his Wolverine's talons to his 1-Man Legend Show tuxedo.  Other venues include birthday parties, special events, reunions, dinner shows, night clubs, grand openings, corporate entertainment, etc.

No stranger to the big stage, Michael has opened for Las Vegas acts such as Al Martino, Bobby Vinton and Connie Francis.  Mike still keeps the memory of his favorite performance as the opener for the Scintas Las Vegas show at the Sahara Hotel and Casino.

Michael is an actor, model as well as a vocalist. He is also a songwriter and recording artist. He has performed on radio and television, in movies and on stage. As a model, Michael has been seen in many prestigious magazines, i.e., HOUR Magazine, Michigan Entertainment (ME) Magazine; Fitness commercials, Providence Hospital medical network.

Michael's most recent film performance was in "Rockwell" as the lead character, Benson Rockwell, a Michigan independent film production, produced by award winning producer Joshua Amir.
